COVID-19 Vaccination Clinics

 

We are currently accepting requests for those that meet the current guidelines set forth by the Governor. If you submit a Request and do not meet the criteria your request will NOT be accepted.

 

All previous submissions have been added. Therefore, no need to contact the hospital for confirmation.

 

The Request Form is located under the COVID-19 Vaccine Tab in the Drop Down Menu. 

If you have been registered for an appointment, please: 

  • Do not enter the building 5 minutes prior to your appointment. 

  • Please enter through the Main Hospital Entrance. 

  • Bring a form of identity and Insurance Card. 

  • There will be reserved parking. 

  • Upon entering the Hospital, you will be escorted to the clinic area. 

Phase 1B Timing

Vaccinations in Phase 1B are expected to begin the week of January 19. Governor DeWine announced a tiered system for offering vaccinations to the estimated 2.2 million people who are eligible for the vaccine under this phase, beginning with those who are 80 or older. When a new age group begins, vaccinations may not be complete for the previous age group. It will take a number of weeks to distribute all of the vaccine given the limited doses available.
The week of Jan. 19: Ohioans 80 years of age and older.
The week of Jan. 25: Ohioans 75 years of age and older; those with severe congenital or developmental disorders.
The week of Feb. 1: Ohioans 70 years of age and older; employees of K-12 schools that wish to remain or return to in-person or hybrid models.
The week of Feb. 8: Ohioans 65 years of age and older.

 

Phase 1C Timing

The entire 1C vaccination group can begin their vaccinations on Thursday, March 4. This includes certain professions and those with certain medical conditions not covered in previous phases.

 

Phase 2 Timing: 

Also beginning March 4th, Phase 2 of our vaccination program will begin, with a lowering of age eligibility to 60 years of age.
